The department issued show-cause notices, directing the nurses to report to the department immediately.

The notices were served to Nurse Nazia Hassan, Nurse Sunaina Khalid, and Nurse Hafiza Saima Sahar, accusing them of negligence in preparing the injection solution.

According to the notice, their alleged carelessness resulted in an adverse reaction affecting 16 patients in the Chest Ward. The nurses have been ordered to submit a written response within seven days.

The case has escalated after reports of fatalities linked to the incorrect administration of injections.

Sources said that an internal investigation report was completed last night but was allegedly suppressed.

The Young Nursing Association has announced protests, arguing that nurses are unfairly blamed when all medications are administered under pharmacists’ instructions.

Hospital sources confirmed that two patients — a woman and a man — died due to the suspected injection reaction.

In total, 18 patients reportedly suffered severe complications, including high fever, chills, and breathing difficulties.

Mayo Hospital’s medical superintendent stated that the reaction occurred in the Chest Surgery Ward, raising concerns over the hospital’s medication handling procedures.

Initial investigations suggest that hospital staff may have mixed the powdered injection with the wrong solution, possibly using Ringer’s lactate and calcium gluconate instead of sterile water.

Punjab Health Minister Khawaja Salman Rafique assured that the investigation report would be made public soon.

He vowed strict action against those found responsible and announced an audit of all government hospitals to prevent similar incidents in the future.
